We are looking for a Diabetes Practice Nurse to join a leading provider of health improvement services across the UK, based in King's Lynn. This Diabetes Practice Nurse role comes with a salary of up to £44,000, a £5,400 car allowance, excellent training and development opportunities, and a range of additional benefits.

Diabetes Practice Nurse Benefits include:

Monday–Friday, 9am–5pm schedule

This Diabetes Practice Nurse role is an exciting opportunity to make a real impact in the lives of people with Type 2 diabetes. You’ll be part of a dynamic, supportive team committed to delivering proactive and patient-centred care in primary care settings. If you were already in this Diabetes Practice Nurse role, here are some of the things you’d be doing this week:
 
Delivering care in line with national diabetes guidelines
Supporting patients with sub-optimally controlled Type 2 diabetes
Conducting point-of-care testing and insulin referrals
Providing patient education and lifestyle advice
Collaborating with GPs and the wider healthcare team to ensure optimal care
Fulfilling corporate nursing responsibilities
We are looking for a Diabetes Practice Nurse with:
A Diabetes Diploma or equivalent ENB qualification
At least 3 years' experience managing diabetes in primary care
Strong knowledge of oral hypoglycaemic management
Confidence in delivering patient education and support
A valid NMC pin and full UK driving licence
